Membership Cancellation Request

To terminate your membership agreement, you MUST fill out this form as this will serve as your written notice. Without submitting this form, we will be unable to proceed with the cancellation process. 

Your notice should be given 28-days in advance.

If there is a payment WITHIN the 28-day notice that must be made, then that payment will be charged and then the membership will be terminated.

In case you decide to reactivate your membership in the future (and we hope you do), membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.

If you would like to put your membership on hold instead of canceling, you can submit a Membership Hold Request instead.
